ENCLOSE your completed Official Offer Certificate, originals or legible copies of all receipt(s) required, and the original UPCs of all products purchased in a single first-class stamped envelope. 5. MAIL all of the above materials in a stamped single envelope to the address indicated on this certificate so that it is received by no later than 1/21/09.
